However, the Magpies fought back to go in front, with Alexander Isak and Anthony Gordon producing similar finishes past Stefano Ortega.
With City losing the game at 2-1, Guardiola summoned De Bruyne from the bench.
The Belgian midfielder pulled the visitors level with a low effort, before setting up fellow substitute Oscar Bobb to grab the winner.
The result takes City up to second on 43 points, two behind leaders Liverpool.
