Prison chief resigns over botched execution of de@th row inmate Christa Pike who survived two lethal injections

The commissioner of the Tennessee Department of Correction, Frank Strada, is set to resign following the failed execution of death row inmate Christa Pike, who survived two attempts to administer lethal injections and was left critically ill.

Tennessee Governor Bill Lee announced Strada’s departure on Saturday, October 3, as an independent review gets underway into the failed execution at Riverbend Maximum Security Institution in Nashville. Strada said stepping aside was in the best interests of Tennessee and described the investigation into the incident as “entirely appropriate and necessary”.

The failed execution on Wednesday left Pike unconscious, intubated and dependent on a mechanical ventilator. Her lawyers said she remains critically ill in hospital. Strada said the Department of Correction had followed the established execution protocol but accepted that stepping aside was appropriate while the independent review proceeds. He said: “Even so, stepping aside is in the best interest of Tennesseans as the independent review moves forward.

“I believe this review is entirely appropriate and necessary, and I look forward to doing everything I can to help determine exactly what occurred.” Lee said Strada had “served with integrity” and praised his decision to put Tennesseans first during what he described as a “difficult moment”.

The governor said he had accepted Strada’s resignation and thanked him for helping ensure a smooth leadership transition. According to a court filing by Pike’s lawyers, medical staff made repeated attempts to establish intravenous access before administering two doses of pentobarbital.

Her attorneys said at least seven needles were used in her left arm alone and that one appeared to have been bent after being removed following an unsuccessful attempt. Pike reportedly tried to help the medical team locate a vein during the attempts to establish the IV lines. Her lawyers said she cried, whimpered and breathed loudly throughout the procedure. At one point, Pike reportedly complained: “My arm feels like it’s about to burst open.”

The lethal drug was eventually administered, but the execution did not result in her death. Pike was taken to a hospital in the Nashville area, where doctors have been treating her and working to clear the drug from her system. Her lawyers have alleged that the IV lines were improperly placed or that her veins had blown, causing the pentobarbital to enter surrounding tissue rather than her bloodstream.

They wrote in a court filing: “Apparently at no point did any member of the execution team realise that the IV lines were not correctly placed or that the veins had blown and that the pentobarbital was, in whole or in part, entering Ms Pike’s body.” Attorney Randy Spivey said: “I counted at least seven needles used in her left arm alone. “One of which appeared to be bent when it was pulled from her left arm after an unsuccessful attempt.”

Pike’s lawyers had previously warned that she had small veins and a blood condition that could make intravenous access difficult. They had asked Tennessee to allow alternative execution methods, including a firing squad or hanging, and requested that an independent physician and an intensive-care ambulance be available. Her attorneys said their concerns had been “laughed off, scoffed at, and dismissed” as “speculation”. Pike’s lawyers are now seeking to have her death sentence commuted following the failed execution.

Medical experts have also raised concerns about possible long-term consequences from the ordeal. Dr Joel Zivot, an Emory University School of Medicine professor who has been involved in Pike’s case, told the BBC that she could have suffered a brain injury. “It’s very possible that as a consequence of the delay of the beginning of resuscitation she will have a brain injury,” he said. Another doctor, Ferrell, said it remained too early to determine the extent of any injuries or how much of the ordeal Pike may remember.

He told People: “It’s such an unknown: what was conscious, what will be remembered, what damage may have occurred to her brain from oxygen deprivation. We don’t know at this point, where we are, everything we say, I think, would be speculation. But we may never know what all she was suffering through, either because she can’t remember it, she can’t tell it, or it overwhelms her in such a way that sometimes pain and suffering affect people that way.”

Pike has spent three decades on death row after being convicted of the 1995 murder of her 19-year-old classmate, Colleen Slemmer. She was sentenced to death in 1996. Had the execution succeeded, Pike would have become the first woman executed in Tennessee in more than 200 years.

Strada joined the Tennessee Department of Correction in 2023 after previously working in Arizona’s prison system. According to the source report, he oversaw three lethal injections while working in Arizona that were later criticised over problems with the executions. The failed execution was the second in Tennessee this year, prompting Lee to halt further executions in the state for the remainder of 2026 and order an independent review.
